PATCHS

Our new online consultation software called PATCHS goes live on Monday 6th February 2023. Please click the PATCHS banner below to access PATCHS and submit your requests online.

Please note that PATCHS is an alternative method of communication with us rather than having to phone the practice. You can request an appointment for any of our clinicians or other health related information such as blood test results or request a sick note. PATCHs does not have the capability to book an appointment for you automatically. Your PATCHs request will be assessed by a member of our practice team and you will receive a text message advising you of your management plan. This might be a link for you to book an appointment at the practice or at another health facility or it may give you advice and signposting. When all available slots have been booked, PATCHs will not accept further requests for an appointment on the system. If you need our help urgently that day, then you should ring the surgery as you normally would.

NEW PATCHS Telephone Assistant

Moorcroft Medical Centre is pleased to offer a new automated service for our patients – we have listened to patients concerns about long phone queues and waiting times – this new option means that in 4-5 mins you can complete a request and leave the rest to us.

Patchs telephone assistant will be available Monday to Friday from 8am to 1pm.

Patchs telephone assistant has proved very popular with many patients, they find it a quick and easy way to communicate with the practice and not have to wait in long telephone queue.  This service will be an easier way to request non-urgent appointments and make administrative queries with the practice.

The Automated Service will now be used to support patient requests, offering a quick and easy service.  All requests will be reviewed, and patients will be contacted on the same day which may include a future appointment, advice, or signposting to other services.

Frequently Asked Questions (FAQs)

How will I know that my request has been submitted to the surgery?

After completing the questions, you will be advised that your request has been successful and submitted to Moorcroft Medical Centre.

Will I need to create an account and have a login and password?

No, just call the surgery on the normal number and select the option for Telephone Assistant. Once you have given your date of birth and name the system will find you.

Can I submit a request on someone else’s behalf?

Yes, only if you have their consent to do so.

How will I be contacted?

You may receive a call, text message or a link to book an appointment.

Do I need to know my NHS Number to use this service?

No, all you need is your date of birth and name.

Patient Online Access

You can access the online consultation facility through your Patient Online Access account (if you already have one set up). Please click the Patient Access banner below.

Proxy Access

Proxy access often used by parents of children, recognised carers of young children and recognised carers of adults.

If you require proxy access, please collect a form from reception and follow the instructions on the form. Please return this fully completed with photo ID and proof of address for both parties. 

If someone has child proxy access, please be advised that this is automatically removed at 16 years of age. The person turning of this age will receive a letter from the practice advising them of this. Proxy access can be re-applied as an adult by completing the relevant form. This form can be collected from reception.
